Evidence Based Screening Case Study Christopher Mason Clinical scenario Miss. J, (not the patient’s real title or Initial), is a 37-year old female who was recently admitted to the hospital for chronic fatigue leading to exhaustion. As a career she says, â€Å"I am a professional writer and aspiring actress.† She lives alone on the Westside of Los Angeles in a one bedroom apartment. During our conversation she disclosed that after a five year period of not smoking; she has begun smoking again. Miss. J says that the stresses that come with her career path as an actress and writer along with the people involved in the same has most likely caused her to do so. Miss. J denies any illicit drug use but does occasionally use alcohol as a stress reducer. As far as she knows no one in her family has died from or developed any type of cancer. Her father passed away from Acute Myocardial Infarction and was a type 1 diabetic. Her mother has steroid induced type 2 diabetes. Miss. J says, â€Å"I have managed to avoid both. So far.† Identification of the Evidence Based Guideline Based on the interaction with Miss. J, several risk factors became apparent and they include a family history of diabetes, type 1 and type 2. Even with type 2 diabetes having a strong familial association her mother’s was induced by steroids rather than being genetic. The other risk factor that I identified was the smoking of cigarettes and casual use of alcohol as a stress reducer. This places her at an increased risk for use of other illicit drugs, driving accidents, lung cancer, COPD, and high blood pressure, as well as other smoking related diseases. Reason for Selecting Guideline Based on the United States Preventive Services Task Force (U. S. Preventive Services Task Force, 2010), being screened for high blood pressure is recommended as an â€Å"A† level screening. The current level defining hypertension in adults is equal to or greater than 140 for systolic and equal to or greater than 90 for diastolic. Based on variability between separate readings, it is recommended to obtain greater than 2 readings on different visits over several weeks to diagnose hypertension. (p. 35) Barriers and Facilitators for Client Miss. J has only one barrier that I could pinpoint to following up with the screening. Her fatigue. She told me that sometimes she misses appointments because she is too exhausted to drive to them. She agreed that she would make an appointment with her personal physician after being released from the hospital to be screened for hypertension. Adding that she just hoped that she would be able to make the appointments. One of the facilitators that allowed me to participate in a conversation with Miss. J concerning her hypertension was her stating to me that she wished she could have a smoke. That along with her being a captive audience in the hospital. No one had been to visit with her that I had seen, so I assumed and was correct that she would like to chat. Receptivity of Client Miss. J was receptive to the screenings I have listed below. She stated to me that she had recently had a pap smear and tested negative for abnormal cervical cells and HPV. Being asked about the Chlamydia screening because of being sexually active; she said it was something she was interested in receiving more information about. Evidence Based Screening Guidelines Pertinent to Patient Type Clinical Considerations and Currency Situation Cervical Cancer Women 21 to 65 (Pap smear) every 3 years or, for women age 30 to 65 years who want to lengthen the screening interval, screening with a combination of cytology and HPV testing every 5 years. Grade A+ Recommended. (Screening for Cervical Cancer, 2014). Yes, she is current. Chlamydia All sexually active non-pregnant young women aged 24 and younger and for older non-pregnant women who are at increased risk. Grade A+ Recommended. (Section 2. Folic Acid All women planning or capable of pregnancy take a daily supplement containing 0.4 to 0.8 mg (400 to 800  µg) of folic acid. Grade A+ Recommended. (Folic Acid for the Prevention of Neural Tube Defects, 2014) Yes, she takes daily supplement. HIV Adolescents and adults aged 15 to 65 years. Younger adolescents and older adults who are at increased risk should also be screened. Grade A+ Recommended. Tobacco Use This recommendation applies to adults 18 years or older and all pregnant women regardless of age. The USPSTF plans to issue a separate recommendation statement about counseling to prevent tobacco use in non-pregnant adolescents and children. Grade A+ Recommended. Healthcare Provider Barriers I am performing my clinical rotation at Cedars Sinai in Beverly Hills. They do not allow us to use electronics in or around the patients/clients. After explaining to my clinical liaison about our subject and assignment we were cleared to use the program as long as we did it outside of the patient’s rooms. This was limiting but not considered a complete barrier. Confidence in Providing Evidence Based Screening Guidelines This assignment made me feel uncomfortable at first. I realized that I would be asking some very personal questions to a patient that I did not know. Three Types of Dieters A sad fact in American society is that thousands of people search for the elusive dream of being thin. On any given day, one finds neighbors, friends, and relatives on some kind of diet. Dieters assume various disguises, but the noteworthy ones are the "bandwagoneer," the "promiser" and the "lethal loser." Everyone wants to lose weight quickly and effortlessly; therefore, any fad diet promising overnight results becomes the new "call" of the "bandwagoneer." She tries the grapefruit diet or the watermelon diet, but she decides her stomach cannot possibly deal with all of that fruit. The next day the television advertises a new wonder pill that allows the user to lose up to ten pounds in one week, and the "bandwagoneer" answers the "call." Although the magic pill does not produce the desired weight loss, she never gives up hope for a new "wagon" to hitch onto. Once again, this dieter is lured by advertisements of instant spot reduction--liposuction. She crosses over the safety line into a danger zone of unknown procedures, performed by unqualified physicians. Some dieters lose their lives in the search for a beautiful body. The stomach staple is another dieting tool that dieters try. The staple yields a large weight loss, but the dieter endangers her health because of excess loss of body fluids. The "bandwagoneer" is always listening for the newest cure on the dieting market. A family wedding or a special dance is a logical reason for a woman to decide it is time to take off her few, unwanted pounds; however, decisions made in haste are hard to keep, and the "promiser" soon fails in her attempt. She is the dieter with only fifteen pounds to lose, and, as each year flies by, she decides dieting is harder than eating what she wants to, and much less fun! She promises to lose the extra weight for her ten-year class reunion, but her weight-loss pledge is not kept. Some women become "promisers" during their pregnancies, and they broadcast to all within hearing distance that they will lose the extra pounds as soon as the baby is born. The "tomorrow promiser" and the "Monday promiser" are the dieters with whom most people are familiar and whose excuses they know.

Native Americans in the United States and Squanto Essay

Squanto is a Native American who lived in the early seventeenth century in what is now the Northeast United States. When the English came to this area of America to settle, they became very fond of Squanto and used him as a translator due to his unique knowledge of the English language acquired through an earlier voyage to Europe. Squanto helped the Pilgrims adapt to their new surroundings by providing them with the knowledge that he and his ancestors used to survive when they first settled in this area. He became known as a friend to the English and a spokesman for his Native friends (Johnson p. 2). However, in helping the English, Squanto realized the power he had obtained through his position and used it for his own gain against the Native Americans. He helped the English to destroy some Indian tribes and used trickery to obtain undeserved favors from many people in his own tribe. While Squanto was essential to the survival of the English in their American colonies, he betrayed his Native American friends in the process of providing the English with what they needed to survive (Johnson p. 2). Squanto spent much of his life living in the Plymouth Colony teaching his newly acquired English friends how to survive in this foreign land. He helped them greatly in the area of growing and gathering food. Without the help of Squanto, the English never would have discovered many important methods involved in growing a decent crop on the American soil. â€Å"Squanto showed the immigrants how to plant corn in hillocks, using dead herring as fertilizer†() after many failed attempts of growing while using their own methods. He also taught them how to fish and where the best spots were to catch enough to feed the colony. These important tips were essential to the survival of the English. Squanto was also very helpful in establishing peaceful relations between the colonists and Indians. In his first meeting with the Plymouth colonists, Squanto was able to work out a peace treaty in order to keep either side from hastily attacking the other (Johnson p. 2). He also acted as an interpreter and a guide to help make the English more comfortable around the indigenous people (). â€Å"Without Squanto’s help, the Pilgrims would probably have had severe famine over the next year, and would have lived in constant fear of their indian neighbors†(Johnson p. 2); the English could not have survived without the knowledge that Squanto had given them. However, the friendly side of Squanto is traditionally the only one taught to the American public, in which he is portrayed as an extremely friendly Native American who devoted much of his time and effort to helping the European settlers adapt to their newly acquired land. It is true that Squanto was a great help to the English settlers, however many modern texts, articles, and essays concerning Squanto neglect to include his betrayal of many Native American tribes in what is now Connecticut and Long Island(Vine p.201 ). Many elementary educated students are led to believe that Squanto simply â€Å"saved the newcomers from starvation ? and [taught] the Pilgrims how to survive†(David p. 24). This misconception of Squanto is the exact opposite of what many Natives thought of him(Vine p. 201). He became aware of his position with the English and began to threaten the indians by telling them he would â€Å"release the plague† (Johnson p. 2) on them if they did not do what they were told. He also provided the English with certain secrets that allowed them to easily defeat other Native tribes and control their land. Squanto was a key factor in the downfall of many tribes of that area, and he is not nearly as great a man as he is traditionally portrayed. From an English point of view, Squanto could be considered a hero due to the importance of the information he provided for the English. Had he not helped them so graciously, they may have died during their first winter. Despite his great deeds toward the English, Squanto could also be considered a traitor. He betrayed his Native friends only to gain power for himself and also for the English. Although knowledge is not something that can be seen, it holds tremendous value to those who are capable of having it. In George Orwells Animal Farm, the downfall of the farm is caused by the unequal possession of knowledge amongst the animals. This possession of knowledge goes hand-in-hand with those who control leadership and power, life and death, and manipulation. The pigs achieve their superiority and power through their academic abilities. Due to the pigs already having more knowledge than the rest of the animals in the beginning of the novel, they automatically take their positions as the leaders of the Animal Farm, The pigs [do] not actually work, but [direct] and [supervise] the others. With their superior knowledge, it was natural that they should assume the leadership (Orwell 11). In turn, their positions as higher authorities allow them to gain access to Mr. Jones farmhouse, using his childrens old spelling book to teach themselves how to read and write. Having gained these skills, and examined the concepts of economics, Napoleon suggests that they engage in trade with the neighbouring farms in order to obtain certain materials which were urgently necessary, mainly rations. Napoleons knowledge of the trades gives him the ability to order Mr. Whymper to acquire rations for the animals and continue to report that there was no food sho rtage on the farm. Thus, with having no other contact with humans while allowing rations into the farm, as well as implying to the outside world that there is no food shortage, Napoleon has demonstrated to both the parties, that he is capable of running the animal farm without any humans, justifying his position as a powerful leader. This, in turn, begins the downfall of the Animal Farm. The other animals on the farm, however, due to their lack of intelligence, have only been led to their downfall. Many are unable to completely learn the alphabet while others do not feel the need to learn to read or write at all, Clover learnt the whole alphabet, but could not put words together. Boxer could not get beyond the letter DNone of the other animals on the farm could get further than the letter A (Orwell 13). The sheep, hens, and ducks, were unable to learn the Seven Commandments by heart. Their inability to effectively read or write stops them from averting Boxers murder, when they cannot read the sign on the van that takes him away (save Benjamin): Fools! Fools! shouted Benjamin, prancing round them and stamping the earth with his small hoofs. Fools! Do you not see what is written on the side of that van?Horse Slaughterer and Glue Boiler Do you not understand what that means? They are taking Boxer to the knackers (Orwell 47). A spark of realization has occurred, and the animals soon understand the importance of being able to read and write. As they watch their fellow comrade being driven towards his death, their guilt has consumed them, for it is their illiteracy which has caused the incident. Had the pigs taught the animals how read or write with ease, or had the animals taken the effort to learn to do so, Boxer would have been spared from this terrible incident. Through their extensive knowledge of reading and writing English, the pigs are able to easily manipulate the other animals minds through the changes made to the Seven Commandments. Squealer manipulates the minds of the animals by using a method called gas-lighting, a form of mental abuse in which information is twisted or spun with the intent of making victims doubt their own memory. When the changes made to the Seven Commandments are questioned, Squealer simply replies that there was no such thing written, questioning their memory, completely aware of their inability to read the Seven Commandments in the first place: Squealer [asks] them shrewdly, Are you certain that this is not something that you have dreamed, comrades? Have you any record of such a resolution? Is it written down anywhere? And since it was certainly true that nothing of the kind existed
